By contacting the Boyd Therapy Center, I acknowledge and understand that email communications over the internet are not entirely secure.

While it is unlikely, there is a possibility that information I include in an email could be intercepted and read by parties other than the intended recipient.

Additionally, I give permission for a counselor from Boyd Therapy Center to coordinate initial contact with me. I consent to them leaving a message at the phone number or email address I have provided.
